How To Register

Mail or Fax
To register by mail or fax; DOWNLOAD the registration packet. Please complete the Registration, Permission Slip & Release Form, and Authorization for Treatment of Minors in its entirety and send it to 6ft Studios. If registering by mail you may send a personal check for the first month payment or pay in full. Online payment is available through the webstore if you are using fax or would like to use a debit or credit card. Our fax number is 616.604.1098. Registration is not complete until both the registration packet and the payment have been received.
